They can occur when a person feels<strong> overwhelmed by persistent negative emotions and thoughts<\/strong>, such as the idea that others would be better off without them. Suicidal thoughts can be associated with mental disorders, but can also occur in response to life difficulties. These thoughts may evolve into<strong> concrete planning and\/or self-harm <\/strong>and sometimes into a <strong>suicide attempt,<\/strong> which may or may not be fatal. <sup>[1]<\/sup> It is important to note that not all suicide attempts necessarily reflect a genuine wish to die. Some studies suggest that suicidal people are often ambivalent about whether they want to live or die and are in fact seeking <strong>relief from their suffering<\/strong>. <sup>[2], [3]<\/sup> At times, someone may act impulsively and lose their life, even though, deep down, they would have wished to continue living. Access to emotional support at the right time can prevent suicide.     <\/p>\n\n<h2 id=\"Frecventa\" class=\"wp-block-heading\">How common is attempted suicide?<\/h2>\n\n<p class=\"wp-block-paragraph\">Every year, approximately <strong>720,000 people die by suicide<\/strong> globally. <sup>[4]<\/sup> Suicide is the leading cause of death in many European countries for young people aged <strong>15-24<\/strong>, representing <strong>17.6%<\/strong> of reported deaths in this age category in high-income countries. <sup>[5] <\/sup>In addition to these deaths, <strong>many more people experience suicidal thoughts or have suicide attempts<\/strong>. It is estimated that for every person who dies by suicide, <strong>another 20 people have at least one attempt<\/strong>. <sup>[4]<\/sup> The prevalence of suicidal thoughts is even higher, given that not all suicidal thoughts lead to attempts. <\/p>\n\n<p class=\"wp-block-paragraph\">In Europe, the average suicide rate is <strong>14.1 per 100,000 inhabitants<\/strong>, above the global average of <strong>10.7 per 100,000<\/strong>. <sup>[7] <\/sup>The highest rates are in <strong>Eastern and Central Europe<\/strong>, followed by Western and Northern Europe, while the lowest are in the Mediterranean area. In <strong>Romania<\/strong>, the frequency of suicide is close to the European average.  <\/p>\n\n<p class=\"wp-block-paragraph\">Furthermore, it is estimated that approximately 135 people in the life of someone who dies by suicide are affected by the suicide, bringing the total number of people affected by suicide annually to approximately 100 million. <sup>[8] <\/sup><\/p>\n\n<h2 id=\"Cauze\" class=\"wp-block-heading\">Causes and risk factors<\/h2>\n\n<p class=\"wp-block-paragraph\">Suicide is a complex phenomenon, influenced by a combination of genetic, psychological, social, and cultural factors. These factors can interact with each other and increase the risk of suicide <sup>[8]<\/sup>: <\/p>\n\n<ul class=\"wp-block-list\">\n<li><strong><mark style=\"background-color:var(--white)\" class=\"has-inline-color has-black-color\">Presence of mental disorders<\/mark><\/strong> (depression, anxiety, schizophrenia, ADHD, autism, bipolar disorder, personality disorders, PTSD, etc.);<\/li>\n\n\n\n<li><strong>Suicidal ideation;<\/strong><\/li>\n\n\n\n<li><strong>Previous suicide attempt<\/strong>: this is a <strong>major warning sign<\/strong>, as it significantly increases the risk of a future lethal attempt;<\/li>\n\n\n\n<li><strong>History of self-harm<\/strong>: a study from the UK shows that women with a history of self-harm have a 15 times higher risk of dying by suicide compared to those without such a history <sup>[6]<\/sup>;<\/li>\n\n\n\n<li><strong>History of physical and\/or sexual abuse;<\/strong><\/li>\n\n\n\n<li><strong>Substance abuse;<\/strong><\/li>\n\n\n\n<li><strong>Family history of suicide;<\/strong><\/li>\n\n\n\n<li><strong>Living alone;<\/strong><\/li>\n\n\n\n<li><strong>Age<\/strong>: adolescence or old age;<\/li>\n\n\n\n<li><strong>Gender<\/strong>: although women have more attempts, the rate of completed suicide is 3 times higher in men in high-income countries and 4 times higher in low- and middle-income countries <sup>[5]<\/sup>;<\/li>\n\n\n\n<li><strong>Stress related to belonging to a minority group<\/strong> (ethnic, sexual orientation, migration status, etc.) <sup>[4]<\/sup>;<\/li>\n\n\n\n<li><strong>Major life events<\/strong>: bereavement, serious illness, significant changes (retirement, redundancy, family expansion, relocation), financial or housing difficulties;<\/li>\n\n\n\n<li><strong>Feelings of<\/strong> <strong>inadequacy, loneliness, or isolation;<\/strong><\/li>\n\n\n\n<li><strong>Cultural pressure<\/strong> (e.g. forced marriage);<\/li>\n\n\n\n<li><strong>Side effects of certain psychiatric medications;<\/strong><\/li>\n\n\n\n<li><strong>Trauma of various types<\/strong>.<\/li>\n<\/ul>\n\n<p class=\"wp-block-paragraph\"><strong>The strongest predictors of death by suicide <\/strong>are<strong>:<\/strong><\/p>\n\n<ul class=\"wp-block-list\">\n<li>advanced age,<\/li>\n\n\n\n<li>suicidal ideation,<\/li>\n\n\n\n<li>previous suicide attempt.<\/li>\n<\/ul>\n\n<h2 id=\"Tipuri\" class=\"wp-block-heading\">How does one get from ideation to action?<\/h2>\n\n<p class=\"wp-block-paragraph\">Suicidal ideation is of two types: <\/p>\n\n<ul class=\"wp-block-list\">\n<li><strong>Passive suicidal ideation<\/strong>: the person experiences thoughts about wanting to die, but the thoughts do not take the form of a concrete plan to act in that direction.<\/li>\n\n\n\n<li><strong>Active suicidal ideation:<\/strong> thoughts turn into concrete intentions, and the person plans ways in which they could end their life. <\/li>\n<\/ul>\n\n<p class=\"wp-block-paragraph\">Passive suicidal ideation does not, however, eliminate the risks regarding a suicide attempt. When thoughts of this nature are present, the <strong>transition from thought to action<\/strong> can occur either <strong>slowly<\/strong> or <strong>suddenly<\/strong>. A significant number of people make the decision to take their own life impulsively, without long premeditation. Therefore, it is important not to neglect the signs when they appear.    <\/p>\n\n<p class=\"wp-block-paragraph\">Why do some people end up dying by suicide or having attempts and others do not? One of the dominant theories, called the <strong>Interpersonal Theory of Suicide <\/strong><sup>[9]<\/sup>, argues that suicidal thoughts arise when two elements are present simultaneously: <strong>a lack of social belonging<\/strong> and the <strong>perception of being a burden<\/strong>, which can be accentuated by a <strong>lack of hope<\/strong> that the situation will improve. Although suicidal thoughts are a necessary condition, they are not sufficient for an attempt. According to the theory, the <strong>capacity<\/strong> <strong>to make a suicide attempt <\/strong>develops gradually through habituation to physical pain and frightening experiences, which can lead to a decreased fear of death through its normalisation. The presence of these three elements (lack of belonging, feeling of being a burden, and capacity) represents the condition through which ideation becomes a suicide attempt.    <\/p>\n\n<h2 id=\"Tratament\" class=\"wp-block-heading\">How do we know if someone is suicidal?<\/h2>\n\n<p class=\"wp-block-paragraph\">It is difficult to tell if someone is struggling with such thoughts, but there are a few signs that may indicate a person is contemplating suicide. Many of these signs can only be observed through direct conversation, as they relate to that person&#8217;s perspective and experience. From the outside, some situations may seem minor or not that painful, but for the person living them, the impact can be profound. That is why, if we want to be there for someone, it is important to remain open and talk about how they see and feel things. Some examples of signs include:    <\/p>\n\n<ul class=\"wp-block-list\">\n<li>The person avoids social interactions and isolates themselves from loved ones;<\/li>\n\n\n\n<li>The person acts in a risky manner that could endanger their life, such as speeding while driving;<\/li>\n\n\n\n<li>The person experiences extreme changes in behaviour (may be very anxious or agitated);<\/li>\n\n\n\n<li>In conversation, the person mentions:\n<ul class=\"wp-block-list\">\n<li>pain that seems overwhelming and unbearable;<\/li>\n\n\n\n<li>constant negative thoughts;<\/li>\n\n\n\n<li>loneliness;<\/li>\n\n\n\n<li>feelings of worthlessness or hopelessness;<\/li>\n\n\n\n<li>the feeling that they are a burden to others and that everyone would be better off without them;<\/li>\n\n\n\n<li>difficulty understanding why they think or feel this way;<\/li>\n\n\n\n<li>difficulty imagining another way to cope with problems (besides suicide);<\/li>\n\n\n\n<li>death.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n<p class=\"wp-block-paragraph\">Sometimes the signs are more subtle. Even if they don&#8217;t mention it directly, a person thinking about suicide may leave certain clues: <\/p>\n\n<ul class=\"wp-block-list\">\n<li>They no longer take care of their appearance, their home, or their responsibilities at work\/school;<\/li>\n\n\n\n<li>They no longer enjoy things that they enjoyed in the past (their own hobbies, passions);<\/li>\n\n\n\n<li>They joke or frequently make ironic comments about their own death;<\/li>\n\n\n\n<li>They frequently share memes related to depression or the meaninglessness of life;<\/li>\n\n\n\n<li>They make allusions that they will not be around for upcoming events (for example, an exam or an outing with friends);<\/li>\n\n\n\n<li>They share songs that have suffering or suicide as a theme or listen to artists who are primarily associated with these themes.<\/li>\n<\/ul>\n\n<p class=\"wp-block-paragraph\">Taken in isolation, these manifestations are not enough to signal a problem. For some people, dark humour and sad music are ways of coping with hardships, helping in some cases to diminish certain symptoms of mental disorders, including suicidal ideation. If they come to represent the majority of someone&#8217;s online presence or appear in combination with other elements from the lists above, <strong>it is important to ensure that the person has the necessary support<\/strong>.   <\/p>\n\n<div style=\"height:22px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n<div class=\"wp-block-group is-content-justification-center is-nowrap class=\"wp-block-spacer\"><\/div>\n\n<h2 id=\"Actiune\" class=\"wp-block-heading\">Where can I get help?<\/h2>\n\n<p class=\"wp-block-paragraph\">Suicidal thoughts or feelings can be truly overwhelming and can make you feel like no one can understand or help you. However, there are options to help you cope in such situations.  <\/p>\n\n<h3 class=\"wp-block-heading\"><strong>1. Talk to a mental health specialist<\/strong><\/h3>\n\n<p class=\"wp-block-paragraph\">We know that in Romania a large proportion of people are afraid of going to a psychologist or psychiatrist. We encounter many prejudices that discourage people and cause them to feel ashamed of the emotions they have or to avoid them. Although reaching out for specialised help can be a difficult step at first, in the long run, this can help you improve your quality of life considerably. If you are experiencing suicidal thoughts, there are <strong>two pathways<\/strong> to access mental health services: <strong>public<\/strong> and <strong>private<\/strong>. On the public pathway, you can access services reimbursed by the CNAS, in the following order: your <strong>GP<\/strong> can give you a referral to a <strong>psychiatrist<\/strong>; they can <strong>evaluate<\/strong> you, offer you <strong>medication<\/strong>, and direct you to a <strong>psychotherapist <\/strong>for therapy. On the private pathway, you can go directly to a specialist (e.g. clinical psychologist, psychotherapist, or psychiatrist) who can guide you towards finding the right treatment. You can start your search on the <a href=\"https:\/\/mentalhealthforromania.org\/en\/service\/map-of-professionals\/\">MHR Specialists Map<\/a>, where you can find over 300 mental health specialists, more than half of whom offer free or reduced-rate services for various vulnerable groups.      <\/p>\n\n<h5 class=\"wp-block-heading\"><strong>How can therapy help you?<\/strong><\/h5>\n\n<p class=\"wp-block-paragraph\">Therapy can help you find the resources to get through difficult periods in your life. A psychotherapist can offer you acceptance, validation, and understanding in a safe space, as well as understand the reasons that lead to the appearance of suicidal emotions, thoughts, or behaviours. <\/p>\n\n<p class=\"wp-block-paragraph\">The psychotherapeutic process is different for each individual, and the goals will be established together with the specialist and adapted to your needs. These goals may include:  <\/p>\n\n<ul class=\"wp-block-list\">\n<li>developing a<strong> safety plan<\/strong> to know what to do in case of relapses;<\/li>\n\n\n\n<li>learning <strong>strategies <\/strong>to cope more easily with the problems you are facing;<\/li>\n\n\n\n<li>increasing<strong> hope<\/strong> and the ability to find reasons to live.<\/li>\n<\/ul>\n\n<h3 class=\"wp-block-heading\"><strong>2. Call an emergency helpline (hotline)<\/strong><\/h3>\n\n<p class=\"wp-block-paragraph\">There are telephone services you can call if you need psychological support. Many are <strong>available 24\/7<\/strong>, are <strong>free<\/strong>, and offer a <strong>confidential service<\/strong>, where the person on the other end is open to listening to you and helping you, without judging you. In Romania you can call:  <\/p>\n\n<p class=\"wp-block-paragraph\"><mark style=\"background-color:rgba(0, 0, 0, 0)\" class=\"has-inline-color has-primary-accent-color\"><strong>1. These may or may not be moderated by a mental health specialist. <\/p>\n\n<p class=\"wp-block-paragraph\">For example, support groups can be useful for those who require more or more frequent support than psychological counselling\/psychotherapy sessions offer, can include useful information and advice, and, last but not least, can help by providing simple confirmation that you are not alone in this struggle and that there are other people going through similar difficulties. In such a space, it might be easier for you to share your thoughts and emotions with people who understand what you are going through.  <\/p>\n\n<p class=\"wp-block-paragraph\">In the online space, you can try the <a href=\"https:\/\/discord.gg\/HvJdJavTga\">Friends of Mental Health for Romania Discord Server<\/a><em> <\/em>or one of the <a href=\"https:\/\/www.facebook.com\/MentalHealthRomania\/posts\/459867512054639\">Facebook groups verified by us<\/a>.<\/p>\n\n<h2 id=\"Sfaturi\" class=\"wp-block-heading\">How can I help myself?<\/h2>\n\n<p class=\"wp-block-paragraph\">If you are in a moment of crisis, it is important to remember that you have several options. Besides the choice of whether or not to stay alive, there is a <strong>third option<\/strong>: stay safe for the moment and you can think about the &#8216;final decision&#8217; later. You can follow these steps to stay safe:  <\/p>\n\n<ol class=\"wp-block-list\">\n<li><strong>Reach out to someone now<\/strong>. You can call someone close to you or an emergency helpline to talk openly about the situation you are in. Try not to stay alone to prevent impulsive decisions.   <\/li>\n\n\n\n<li><strong>Remove danger from around you<\/strong>. This can be any object with which you could harm yourself. Or, if you are in a dangerous place, try to move away and go to a safe one. In everyday life, try to build a safe space around you, with as few &#8216;accessible&#8217; dangers as possible.   <\/li>\n\n\n\n<li><strong>Try to make a list, however short, of things you wouldn&#8217;t want to lose. <\/strong>These can be material (a plant you care about that would die if you didn&#8217;t water it), events (a friend&#8217;s wedding that you would miss), sensations (the smell of your favourite food). Anything, however small, whose loss causes a trace of regret.  <\/li>\n\n\n\n<li><strong>Avoid alcohol, drugs, or medication<\/strong> in quantities larger than those prescribed. These can intensify your feelings and make you act impulsively. <\/li>\n<\/ol>\n\n<p class=\"wp-block-paragraph\">Below are a few other methods you can try if you feel a crisis approaching:<\/p>\n\n<ol start=\"5\" class=\"wp-block-list\">\n<li><strong>Focus on your senses<\/strong> \u2013 try the <a href=\"https:\/\/www.afladespretine.ro\/tehnica-5-4-3-2-1\/\">5, 4, 3, 2, 1 technique<\/a>.<\/li>\n\n\n\n<li><strong>Try a simple breathing exercise<\/strong>, like <a href=\"https:\/\/www.youtube.com\/watch?v=92xTPH7OtLs\">this one<\/a>. <\/li>\n\n\n\n<li><strong>Allow your emotions to come to the surface<\/strong> and cry if you feel the need. If you are angry or frustrated, run or do some exercises, tear something to pieces, or hit some pillows. If you feel disconnected, stimulate your senses: hold some ice cubes in your hands until they melt or take a cold shower.  <\/li>\n<\/ol>\n\n<p class=\"wp-block-paragraph\">The app<strong> <\/strong><a href=\"https:\/\/stayalive.prevent-suicide.org.uk\/\">Stay Alive<\/a> can guide you through the steps mentioned above, offering strategies to stay anchored in the present when you feel overwhelmed by the situation, breathing exercises, and a customisable plan designed to help you rediscover reasons to stay alive. The app can also be helpful to loved ones of those facing suicidal thoughts, providing information and resources. <\/p>\n\n<p class=\"wp-block-paragraph\">In the long term, there are numerous methods that have proven useful in preventing, alleviating, or even stopping suicidal thoughts.<\/p>\n\n<ul class=\"wp-block-list\">\n<li><strong>Create a safety plan<\/strong>, on your own, together with someone close to you, or together with a professional. The plan could include: calming exercises and activities, a list of reasons to live, names and contact details of people who could help you, or helplines. <\/li>\n\n\n\n<li><strong>Be kind to yourself<\/strong>. Imagine a friend you appreciate and accept as they are \u2013 then try to think the same about yourself. Give yourself a warm bath, or wrap yourself in a blanket and watch your favourite film. Do not talk to yourself in a way that would be unacceptable addressed to a friend.    <\/li>\n\n\n\n<li><strong>Make a hope kit<\/strong> <strong>\/ happy box<\/strong>. Fill a box with memories and different things that can offer you comfort and lift your spirits when you feel hopeless. The box can contain anything meaningful and useful to you, such as: your favourite book, favourite quotes, photos from trips or with loved ones, letters or messages, notes, your favourite chocolate, a gift received from someone dear.  <\/li>\n\n\n\n<li><strong>Reach out for social support<\/strong> from family, friends, therapeutic or social groups based on certain interests, etc.<\/li>\n<\/ul>\n\n<h2 id=\"Ghid\" class=\"wp-block-heading\">Guide for loved ones: What can I do if I notice signs in a loved one?<\/h2>\n\n<p class=\"wp-block-paragraph\">The worry that someone close to you might die by suicide at any moment can be painful and truly overwhelming. Whether the person has spoken openly to you about the desire to take their own life or there are certain signs that make you believe they are thinking about it, it is normal to be concerned about the situation and to want to find a way to support them.  <\/p>\n\n<p class=\"wp-block-paragraph\">Often, you cannot know for sure what the best way to help is, but there are certain things you can do:<\/p>\n\n<ul class=\"wp-block-list\">\n<li>encourage the person to seek specialised help as soon as possible, to ensure the best possible chances of recovery and to avoid the risk of self-harm;<\/li>\n\n\n\n<li>encourage them to talk about their feelings;<\/li>\n\n\n\n<li>offer them emotional support;<\/li>\n\n\n\n<li>offer them practical support;<\/li>\n\n\n\n<li>help them make a safety plan;<\/li>\n\n\n\n<li>remove objects they could use to harm themselves, especially if they have mentioned specific things.<\/li>\n<\/ul>\n\n<p class=\"wp-block-paragraph\">Also, if you suspect that a loved one has suicidal thoughts, it is useful to ask them about it.
